Pitted Keratolysis là thuật ngữ dùng để mô tả tình trạng nhiễm trùng da do vi khuẩn ở mức độ bề mặt, chủ yếu ảnh hưởng đến lòng bàn chân hơn là lòng bàn tay. Tình trạng này thường do các vi khuẩn như Kytococcus sedentarius và Corynebacteria gây ra. Nó thường xảy ra thường xuyên nhất ở những người từ 21 đến 30 tuổi, phần lớn trải qua nó ở độ tuổi 20 hoặc 30. Đàn ông có nguy cơ mắc bệnh này cao gấp 4 lần, có thể do đi giày chật, kín thường xuyên hơn, trong khi phụ nữ có xu hướng giữ vệ sinh bàn chân tốt hơn. Pitted Keratolysis is a descriptive title for a superficial bacterial skin infection that affects the soles of the foot, less frequently, the palms confined to the stratum corneum. The etiology is often attributes due to Kytococcus sedentarius and Corynebacterium species bacteria. Pitted keratolysis is most common in the age group of 21 to 30 years, with a majority of affected patients in their 1st to 4th decade of life. Males are at 4 times higher risk of being susceptible to this condition, presumably, due to frequent use of occlusive footwear, whereas females maintain better foot hygiene. We present a case of a 23-year-old medical intern who presented to our hospital with complaints of pitted skin lesion over base of foot, predominantly over toes for past 3 days.

A 30-year-old male presented with small punched-out lesions on the plantar surface. A superficial shaving was carried out for scanning electron microscopy. Hypokeratosis was noted on the plantar skin and in the acrosyringium, where the normal elimination of corneocytes was not seen. At higher magnification (x 3,500) bacteria were easily found on the surface and the described transversal bacterial septation was observed.

Tình trạng này khá phổ biến, đặc biệt là trong quân đội, nơi giày/ủng ướt được mang trong thời gian dài mà không tháo/làm sạch. Chẩn đoán keratolysis rỗ thường được thực hiện bằng cách kiểm tra trực quan và nhận biết mùi đặc trưng. Điều trị sẹo rỗ đòi hỏi phải bôi thuốc kháng sinh lên da như benzoyl peroxide, clindamycin, erythromycin, axit fusidic hoặc mupirocin. Những nỗ lực phòng ngừa nhằm mục đích giữ cho bàn chân khô ráo.

Luôn giữ cho bàn chân và tất của bạn khô ráo. Hãy thử dùng thuốc mỡ kháng sinh OTC. Sử dụng chất khử trùng tay trên đôi chân của bạn cũng có thể hữu ích.
